This survey-based study is looking at how Lyme disease (LD) impacts people’s daily lives and regular activities. It will help us understand LD and its impacts on patients. Those who qualify for the study can earn reward (gift) cards as compensation for their time and effort.


Those who may qualify:

Adults who have recently filled out a prescription from a CVS Pharmacy® for LD and self-reported at least one symptom.

Earn Reward (Gift) Cards

  • In appreciation, participants will earn reward (gift) cards as compensation for various surveys completed during the study. Earn up to $35 reward (gift) cards for taking the first survey and confirming medication start date. Earn up to $175 more in rewards for taking five additional surveys over six months. The reward (gift) cards may be used everywhere debit cards are accepted. Restrictions may apply; see www.mydashcard.com for more details.
